TRYING TO LOCATE CORRECT CARRIAGE PARTS BASES ON CARRIAGE #'s

I am trying to figure out what type of carriage my lift truck has so that I can get the correct seal kits for the the following....

fork positioner cylinders
side shift cylinder

here is a list of the numbers i have

Truck Model # CGP55
SERIAL # CGP460L-0067-9507FB

MAST #'S
453OM1087
HLT00493

The tag on the carriage was worn so I could not make out the manufacterer however the stamped numbers were good, here are the numbers

15000LB 525LB
