Hi all I work in safety and have had an incident I am due to go and investigate. Basically we have had a water gauge crack resulting in boiling water being spraying out of the radiator cap at an operative.

I am being told that it was due to overheating. Does anyone with a technical background have any idea how or what caused this?

Fins could be plugged up. as for hot water on opperator. I d say the opperator removed radiator cap and thats what sprayed him.Not saying it couldnt happen , but usually coolant will come out when hot and cap will stay on and coolant just blows down. If his hands got burned, then he removed cap.

In your case could be from rad as it has hyd oil pressure in the top part of rad

See this happen when inter cooler gone sending engine oil pressure into water system if that's any help

Thermostat stuck shut or radiator cooling fan not running.
