EMB SWIFT drive issues

Hi,
Our Jungheinrich EMB Swift is currently having drive issues in both forward and reverse. The drive wheel spins a lot and leaves black marks on the concrete. Occasionally the wheel wont turn at all but most of the time the motor turns the wheel but it just spins. The wheel not getting traction is worse when there is a load. The wheel is in poor condition some small chunks taken out of it, the drive surface has turned black maybe from spinning and there are some small flat spots. The motor appears to be fine? You can adjust speed/power like normal. The only other issue I can see is the, the mounts for the non-drive rear wheel are not parallel to the wheel and are splayed out like this: /||\
Not sure if this is normal or if this could be adding/causing the drive issues. The mounts are quite solid around 10mm steel so would have to be a heavy knock to bend out like that? Does it sound like I just need to replace drive wheel or could it be something else? Thankyou very much everyone, any help would be really appreciated!
