The U.S. National Institute of Occupational Safety and Health (NIOSH) has recently released the following Fatality Assessment and Control Evaluation (FACE) Report for a forklift fatality that occurred in Massachusetts.

Massachusetts Case Report: 08-MA-019 - Forklift operator crushed between forklift cage and mast while loading waste paper into a trailer.
