(Demerit) Point System for Lifts

Can anyone point me toward a document that lists Lift Operator Infractions and a point system?

When I worked at IKEA they called it PLEPS (Powered Lift Equipment Point System). It outlined various potential behaviours of operators - from non-report of a collision (25 points) to leaving forks above ground level to non-completion of a pre-trip (2 points each).

I am thinking of implementing something like that where I am now - and would rather not "re-invent the wheel" if possible.

That sounds like a solid idea, Steve. You might want to check safety manuals or OSHA guidelines, some companies share similar point systems that could help you build your own.

This is a really old post, but does anyone have any current info about a demerit point system they could share? My operators are all terrified of admitting they bumped into something, so nothing is getting reported. We have cameras, but it's only worth investigating the major incidents, so the small scratches and bumps on our brand-new equipment are piling up.
