Load Handling

We have had the instance of a broken tyne on a 12 tonne forklift. The heel was well within tolerance. The cause we think is continious tip handling on 2440mm long tines with a 4-5 tonne load. Does anyone have info on where there may be information with regards to this practice in order to inform and retrain the operators or to inform the customer of the results cause by this other than the most obvious.

Have done that, what I am looking for is some article or instruction that gives some wieght to my discussion with the customer with regards to the damage carrying loads at the end of forks causes

tolerance standard is 10% loss off material at the heel
so measure the vertical portion's thickness front to back
then measure the thickest part of the horizontal portion
for example : if it is 80mm on the vert than the thinnest the hori part should be is 72mm
there are gauges to measure wear on them

dont let them drag the forks
dont let them hit big potholes at high speed bouncing the product
