I have searched and searched and came up empty. The only statistic I found last year was that there were 21 forklift related deaths in Eastern Ontario over the previous 5 years, with 6 being the greatest amount in any one year. Since I live in Eastern Ontario, I definitely point out that fact, and then knocking on wood, tell my students that I never trained at those facilities.

I would tend to believe, like anything else, take the US figures and divide by 10. lol
