I'm looking at the settings I recorded off the original card, to put in the rebuilt card , and on functions 11,12,13. which are LS1,LS2,LS3. The setting are LS! 145, LS2 000, LS3 180. Assuming LS2 is the switch at the top when forward & reverse is cut out, and the 000 setting meaning it is turned off in the card. I'm thinking mabe the F&R ground coming from PB4 & 5 are going thru relays activated by a ground from a n/c LS2 switch, and when the LS2 switch opens at top highth the relay is deactivated cutting out ground to the F & R coils to eliminate drive at certain level. It would explain the no fault code, because this would be a normal function in operating lift. When I get back there I will check for broken wire to LS2 or bad switch.
It sounds pretty desperate, Why it may work.
