Looks like the manufacturer was in Northern California. The ITA (Industrial Truck Association) should have more info.
Unless there was a serious accident OSHA usually will respond to an appeal.....or you could sell it and buy something else with a data spec plate.
I guess you have to decide if you got your moneys worth out of it yet.
