Propane Tank Identification

Good Morning! Is there anyone familiar with the different types of forklift propane tanks? I was recently involved with an incident involving a tank that was sold with a forklift in 2022 that didn't have a fuel indicator guage installed. It only had the bleeder port and the connection valve. The tank was attempted to be modified via the bleeder port and the escaping compressed propane ignited causing major injury to myself and another employee.
I have at least 20 years experience in dealing with propane powered forklift and have never seen a tank without the fuel level guage installed. Could see give me some information on this please. Thank you.
