i had a similar problem to a FBM20 with error code C0 A9 and i find one IGBT bad and replace all 3 because on component tester give one complet bad and the other 2 with different resistance value...now is working ok for some time(previously i change the 2 boards).
I pulled all of the diodes and tested them and they were all OK, I pulled the circuit board #28 part #2430-12242-71 (5FBc DC Converter & Sdd/sdp) and found some black spots where it had been hot so I ordered a refurbished board and replaced it, so far it has been working fine.

Hi Rich, We have a Toyota model 5FBCU25 forklift that is having the CO code issue and it doesn't move in forward or reverse but everything else works. The forklift mechanic came and checked everything out and couldn't find any problems and said the transistors were bad. We ordered all 3 new transistors along with the heat transfer silicone and installed them ourselves but they only lasted a month before they went bad again. we replaced them again but only lasted about 30 minuets this time, What else could be causing this issue?
Any chance there is a parts break down with p# for this issue? I think I need to change the diodes on my panel. Transistors blew out twice and we cannot find any shorts or other bad components.

Where are them?they are big diode behine the controller?

we have replaced as well the diode and the capacitor after that it works perfectly well!

What is the main reason for having C0 error code?
We are still not sure if we replaced the defective carbon spring the c0 will not appear anymore.
If you can suggest what other area we have to check so that we can do the
thorough troubleshooting in one time.

Normally brush problems show up as a C9 code. Well good work then Thanks Rich
Mr. Rich, we have found the problem! the carbon brush + spring was damaged of the main motor. Hope this will solve the problem once replace the new parts

weird coincidence had a CO code call this morning no drive, it was forward contactors
At the moment the forklift is down due to "c0" error.
We have found the cause of the error...your right it's the IGBT power thyristors.
the bus bars are well ok. I'll keep you posted about this matter once solve while waiting for the parts. Many thanks Mr. Rich K.
In the back electrical panel behind the cards there are 3 rectangular transistors check those 3 should be infinite one way and contnuity the other way. also check the copper bus bars by the contactors let me know how it goes Rich
