Jungheinrich efg 5.0 ton hose

Hi any idea why the the hydraulic hose of efg jungheinrich 5 ton always comes off the pulley, we replace the pulley & hose, adjust the tension still the hose comes off the pulley, any longer solution cause customer not happy specially with the cost of repair any advice...
