How do you log a defective Forklift.

Previously I have used a Defect book with duplicate sheets. The top white sheet stayed with the truck and the second blue copy stayed in the book. I ve been tasked to look for these books or look for a different way of keeping records of defects. If anyone can help me find these books or if you have a different system can you please advise.
