short run times cause condensation
engine not breathing because of pcv valve can cause it
blown head gasket definitely can cause it
quite a few thing actually
i'd do the oil & filter changing definately
check compression (just to be sure)
clean or change the pcv valve
and try to run the engine till hot more frequently
