you may wont to check the carriage roller bearings and side roller bearings. sounds like you could have some that are bad. Also, Ive only seen this a few times but make sure the channel is not warped. One other thing, you might want to check chain adjustment

A 70-year-old man was killed by a truck while cycling; hours later, his twin brother was killed by a truck on the same road, about a mile away, before the family had been informed of the first accident.
