Dies out past 1/4 throttle, no codes

Hi, new to the forum. I am having some trouble with a Mitsubishi 2fbc25 48v forklift. It will go forward and reverse if you stay under around 1/4 throttle. Once you go past that it just cuts out. You have to turn the key off and back on for it to start moving again. It puts up no codes when this happens.

It started doing it 2 weeks ago, and then all of a sudden it started working fine. It was good for those 2 weeks with daily 24 hour use. It has started doing it again. Looking for any suggestions on where to start. Thanks.

FWIW, we found the sensors in the accelerator pedal were dirty. Cleaned them out and all is working good.
