Has anyone repaired carriage fork mount because of wear?

Yes, When the carriage bar becomes worn and damaged where the hooks engage it is possible to gouge-out and replace the carriage bar itself. Some problem areas can be touched-up with a welder but in most cases welding disrupts the temper of the carriage bar and creates weak areas. Even welding a carriage bar to a carriage is tricky to do properly, at the very least contract a certified welder for this task.
