Broken flex plate

Hello,
What could cause two broken flex plate within 20 hours of run time?The bolts are tight,checked crank end float OK,transmission is not locked up and forklift is never overloaded.It's a Clark model CDP25, ser. P365D with Perkins diesel engine,auto transmission.
I have no idea where to begin.Thank you.

I doubt 2 plates will fit, either way I wouldn't install 2 plates, I'd find the root cause of the problem.
What brand of flex plate was used as a replacement, dealer or aftermarket?

Yes they did,4 small pieces left under torque converter bolts. The fly wheel side plate is still bolted no cracks.Only inside circle of flex plate is damaged.Almost like transmission locked-up and engine kept spinning.The only work done is head gasket about year ago.What do you think of idea to install two plates,double it for more strength?Thank you.

Did they both break at the same location? Where did they break? Why was the original plate replaced-did it break on it's own or was other work done on truck first, then plate broke?
