Forklift rolls, fatally crushes two workers

Investigators for the Occupational Safety and Health Administration (OSHA) are reviewing how an apparently untended forklift fatally crushed two workers in Dallas.
"OSHA is investigating the incident that took place on June 5 at Nationwide Plastics Inc," says Juan Rodriguez, deputy regional director for public affairs in the Dallas area office of the US Department of Labor.
The two men were dealing with a load at the back of a truck, says Jason Evens, public information officer for the Dallas fire-rescue department. "The forklift rolled and crushed the workers against the back of the truck."
Emergency responders were alerted about 6.45 pm.
The workers died at the scene.
Nationwide Plastics distributes sheet, rod, tube and film plastics.
