Turned out that my problem was actually the encoder bearing. I had a aftermarket bearing and it wasn't the correct bearing. Got the proper bearing and machine is back in service.
Hey thanks for your reply. hope your safe from this covid-19. I have done more testing on the machine and I have determined that the motor itself is the issue. Not sure how familiar you are with these machines but before I took the motor apart to replace the encoder bearing the motor worked fine. Is there anything that I could have done wrong putting it together? It an AC motor and its the first one i have taken apart. it seems simple enough. Just a note the encoder bearing is also good.

Does you forklift display any codes?... If not, check you limit switch.
