I would start with a tune up if the unit has been tuned up in a long time.

The 5 in the model number designates the GM engine. If it had a 4 it would be the 4Y.

Sounds like the idle spped or governor needs to be adjusted...what is the idle speed when in park...??? Do you know what 4 cyl engine it is....GM engine or toyota 4Y engine?

4Y: 1250 & 25 rpm
GM: 1050 ~t50 rpm

This is the idle speed setting.......

Could be a number of things.... a LITTLE bit of information would help in an arm chair diagnosis....like full model and serial number of the truck....is it gas or LPG......what engine does it have.....how many hours on it....what are the symptoms, does it do it in park or when in gear...hot or cold engine...???. Be a little more thorough in your expination of what and someone might be able to then tell you why....
