Exhaust valve Failure

Please assist.
We have a truck with an ADE 409 diesel engine.
The engine was rebuilt about two months ago and did about 5000 km's.
Two days ago we had the valve clearance set at the agents.
This morning the engine was "locked"
After stripping we found the exhaust valve on No 3 damaged which in turn damaged the piston and sleeves.
Any suggestions, keep in mind it ran for two months and 5000 km's?

i would be looking at the rebuilders for any warranty on this engine or the 'agents' that did the recent valve clearance settings.

something caused it to drop that valve which shouldn't of happened under normal conditions.
