There are two pictures of a forklift in the September 2007 issue of Popular Mechanics which qualify for What's Wrong with This Picture? They are in an article titled "Blowin' in the Wind" by Jay Leno. The article starts on page 46. The forklift is being used to install a wind turbine for Jay Leno's "green garage".

Google: JAY LENO BUILDS A WIND TURBINE ON HIS GREEN GARAGE to view a video of this activity. The video shows a total lack of understanding among the participants of the hazards and proper use of forklifts. Does this sound familiar?

A Cal/OSHA inspector from the California Department of Industrial Relations, Department of Occupational Safety and Health (DOSH) could have developed writers cramp on the date in question.
