1st start in morning, truck idles fast for 5 minutes then drops to normal idle. Durning the day, if the truck is shut off for 5-15 minutes, or longer, again, the fast idle has to run its course before lowering to normal.

The issue is intermittened, not always happening after 1st start, any ideas?

sounds like a problem with the IAC valve (Idle Air Control)
remove and clean it first
if that does not help you may have to replace it
