EASI Reach Forklift jerking

I have a 6 month old forlift that gets used roughly 10 hours per week. I have been having an issue with it jerking forward when the forks are roughly 12-15 feet in the air. As I boom the carrage out the forklift all off a sudden jerks forward causing me to pull the joystick backward to counter the movement. This action does not happen everytime but it has hapened probably a dozen times since we got it. I have come close to losing a couple of loads and taking out racking because of this issue. It is a huge safety concern as the forklift is jolting forward on its own!

Has anyone experienced this before or know why it might be happening?

Are you saying that when reaching forward the machine jumps forward from the drive motor pushing it?

If the deadman is not pushed down I lose all power to all controls. Maybe it is low on oil, I will have to get a service guy out here to check it out.

?forward tractor motion when reaching out?
sounds like a need to be able to let off the brake/operator presence pedal while operating the reach when looking upwards. The reach cylinders are double action cylinders, meaning they put exactly as much oil back into the tank as they use.
maybe I just don't understand the question/problem well enough.
