We have a Toyota model 5FBCU25 forklift that is having the CO code issue and it doesn't move in forward or reverse but everything else works. The forklift mechanic came and checked everything out and couldn't find any problems and said the transistors were bad. We ordered all 3 new transistors along with the heat transfer silicone and installed them ourselves but they only lasted a month before they went bad again. we replaced them again but only lasted about 30 minuets this time, What else could be causing this issue? I pulled the records and this same issue happened in 2016 and all three transistors were replaced along with the Diode 90892-01022-71 and 2 contactor assy. 24420-13300-71, $5,577.00 worth of repairs

Just had the CO code on my 5FBCU20, turned out to be the negative cable coming out of the battery plug in on the lift...
We received the refurbished board 2-19-2018 and installed it, I unloaded 3 18 wheeler trucks today back to back and the forklift did fine. Thanks for all the help and information!
ordered a board, It will be here 2-27-18 we shall see if that takes care of the problem

all of the cable connectors were checked for tightness and clean, The owner had someone do some testing on the battery and they thought it was in good condition, we only use the forklift to pick up skids and move things and its never used to pull or push anything. I don't think a draw test has been done on the motor. The board # 28 has some black spots on it like its been hot but wasnt sure if that board would cause this issue?
Confirm your battery, connectors and contactors are OK, if battery checks out (load test each cell) examine all cabling for heat or corrosion damage. Perform a draw test on drive motors. Some underlying problem exists to burn out transistors so frequently could be as simple as bad cabling. Is the lift used to push or pull heavy loads?
