Victorian irrigation company Flow Smart Vic is facing two charges under the Occupational Health and Safety Act after a staff member sustained serious injuries when he was struck during the installation of telehandler fork extensions.
WorkSafe Victoria says the company is accused of failing to provide and maintain a safe system of work and failing to provide the necessary information, instruction or training to enable employees to work safely.
“The worker was helping to guide fork extensions on to the tynes of a telehandler when he was hit and fell from the tray of the truck he was standing on, suffering a serious brain injury,” WorkSafe Victoria states.
The accident, which happened in October 2023, is listed for hearing at Sale Magistrates Court this month.
