Toyota with GM 4.3L Vortec - crank bolt

Anybody have had problems with the Front crank bolt coming loose or breaking off? All items were replaced and is still happening.

I had this issue before, check and make sure your engine mounts are not broken or severely worn.

Ray, the engine starts and runs fine. The pulley has been spinning on the balancer. The mating surfaces between the balancer and pulley are not completely flat. Im not sure if the mating surface problem is the cause or effect at this point since I inherited the truck from someone else. I am sure that it is what has caused the bolt to back off and break though.
