oil in coolant

I have a Crown forklift Model C5 with oil in the coolant. Any idea what is the cause.

No cooler is air cooled.

No it was not ran hot. Engine runs great. yes it does have an oil cooler on side of block.

Was it ran hot? Did you check for blown head gasket, or pull a spark plug? I believe these engines have an oil cooler on the side off block that can leak.
