It is a Baker. Otis owned Baker for some time in the 70's and 80's.
The model is most likely an FMD040 or FMD050. The 40 has 7" wide drive tires and the 50 has 8" wide tires. The serial # will be stamped into the frame parallel to the ground towards the front sides of the floor board. The serial # will start with either 18... or 19...
