Thanks for the info on the cicumstances of that incident. I know in hindsight we all can be intelligent after the fact, but does that particular company have "a proceedure manual " ?? a logbook to record a prescribed pre start /afterstart check list ? Then maybe if these items were in place, this accident may not have happened ? Yes logbooks are a pain to some ,but aspects of this accident may have been avoided !
