On the matter of companies that are operating unsafely, the best thing to do is to send them letters - more than one - to management and OHS - and copy the letter to the OHS authorities. In the letter use the words "allege" "prima facie"... and similar to protect yourself. I have done this several times, though you never know if it changes anything.
