Quick check on the relay,swap it iwith the sister truck relay and see if symtom swaps to other truck??????
Like the the thought on the starter relay. We ordered and will try.
It has the 4Y motor. both units and use same starter.

What engine is in this lift & is the sister lift exactly the same model with the same engine?
One thing i would check is the start relay....is it sticking when key is released from start position,if contacts in relay are sticking then will keep the starter engaged longer after key released....just a thought and WAG....
It is being started and stopped a lot but we have a sister truck in the area and has no problems. The bendix on the starter is hanging out very briefly after each start. We think this may be the issue.
With regard to parts the first ring and starter both came from TVH. So the second time around we got them from Toyota. but experienced the same result.
What would cause the starter sprocket to hang out very briefly after each start??????
if the operator shuts off the machine after every pallet taken off a truck, or every time he gets in or out of the operators seat, to would not be odd to start a forklift 56 times a shift, where in your car, your mean daily average is 3 starts a day.
there is good reason that OSHA allows you to -properly- park a forklift and leave it running if you are within 8 meters, are able to see the forklift.
make sure you don't have a key switch that allows restart without going all the way to 'off'.
A couple of things- where are you getting your parts & what is the normal life of the lift- starting every 5 minutes or running all day- limited starting?
