Logging defective MHE

How do you log a defective Forklift.

Previously I have used a Defect book with duplicate sheets. The top white sheet stayed with the truck and the second blue copy stayed in the book. I ve been tasked to look for these books or look for a different way of keeping records of defects. If anyone can help me find these books or if you have a different system can you please advise.

I agree with Budman, we have been producing pre start and safety check logbooks here in Australia for 16 years now and have proved to be very effective , ask the OH&S officer at "PEDDERS SUSPENSION AUST " for confirmation on the effectiveness of using a logbook.

This reply is only for Budcoh. OSHA requires the forklift to be inspected prior to each shift. If there's an accident the first thing the ask for is the Operator Inspection sheets to show proof. An OSHA Representative told me personally that these are preferred to be kept on file for a few years.

1910.178(q)(7)
Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.

I've been in the industry for 28 years. What you've stated in your post will put companies at risk.

Sorry for the late reply, Postage would be about US $8.oo and the book US$30.oo ($31.oo AUD ) So with the AUD gst removed the Total incl postage to the USA would be approx US $36.oo

You need to check this guys location. They are in the United Kingdom, OSHA has no effect there.

I reiterate my previous remarks. Employers should/must cooperate fully with the letter and spirit of the OSHA regulations. However, do not hoist yourself by your own petard by creating unnecessary documentation. Start/begin/create documentation for an unsafe powered industrial truck (PIT) only after the qualified supervisor has determined that the truck is indeed unsafe. Forward the supervisor's determination of unsafe to maintenance who should endorse the request after fixing the problem and return it to the supervisor for file/company documentation. The company should have a carefully prepared (attorney-reviewed) document retention policy and documents should be deep-sixed in accordance with that policy. /s/ dirtft

how many trucks do you have as in the past I have used id-systems an all electronic system ,works well and can access a pre op sheet up to 5 years earlier

We've sent out orders to the USA occasionally upon request. Have a look at our books/kits on our Good to Go Safety site to see if they are any interest - have the added advantage of using a tag to display the date and findings of the last inspection so everyone knows it is up to date, at a glance. website logbooks.com.au has forklift pre start and safety checks in single and duplicate books also there is a book for maintenance. Not sure where your situated but they do post overseas.

There's no OSHA requirement for written documentation of a forklift inspection. If you keep written copies on file you're only providing the OSHA CO with a place to go fishing. Suggest you require the operator to orally notify the supervisor of any problems. The supervisor will then deadline the truck and create a work order to have it fixed. The mechanic will endorse the work order when the truck is repaired. Keep this single document for a specific period of time (30 days) and then deep six it. Draft a documentation standard procedure for keeping and destroying documents and follow it. N. B. Follow all OSHA instructions to the letter and spirit; just don't provide a stack of documentation that might be open to misinterpretation. DIRTFT

google logbooks with a dotcom dot au.com.au after it. You will find what your after on that site, both for single shift and 3 shifts per day and many many more great logbooks.
