I don't have a whole lot of experience with Toyota's but I have seen this before. It turned out to be a bad directional switch assembly. On this model, is it possible to reverse the switches or wiring connections? On the ones I have worked on you can't. Or if you have another same model truck you could try using the directional lever assembly from it. I do not have any Toyota schematics.
