Fork attachment fell off

Need help, what holds attachment in carriage?

On back side of attachment there should be a clip on either side of the bottom of the attachment either bolted on or pull pins
Sometimes the anchors will get bent forward causing your problem

A 70-year-old man was killed by a truck while cycling; hours later, his twin brother was killed by a truck on the same road, about a mile away, before the family had been informed of the first accident.
