fatal accident

A technician fell from a height of about 7 meters and was fataly wounded when the bolt restraining the AWP cage from tipping failed. It seems only a few threads of the bolt were engaged. If anyone has any experiance in the past with a similar failure please contact me, your help in resolving the cause of this accident will be grately appreciated.
