Weirdest thing has been happening to us and has not been easy to duplicate. Battery is fully charged and an operator is going in reverse, the battery meter all of a sudden goes to 0 and the machine coasts without any power to stop. Removing foot from deadman doesn't do anything at all. Driver reported 2 weeks ago and we shut down and powered back up, everything seemed to run normal. This morning it happened again and another operator backed into a pallet rack and injured himself. Has anyone seen this symptom before and have any ideas what it can be?

Ive seen the transport bolts left in on electric brake coils & ive also seen reconditioned brake units assembled incorrectly.
Check the brake disc isnt worn & the airgap is adjusted correctly.
Im not a fan of hydraulic brake systems on forklifts, electric failsafe brakes / hydrostatic drive units everytime for me.
You dont mention what model. Old easi truck has hydraulic brakes and will coast even with key off. Only 7400 has electric brakes that lock up with no power. You say the battery guage shows "0". Sounds like a loss of signal to the ODI. Again, what model do you have? ser #?
Well something is wrong with your system if you have electric brakes, then as soon as you loose power brakes should engage. Brakes should disengage when the deadman pedal is engaged, thus energizing the magnetic coil. MAKE SURE RELEASE BOLTS ARE NOT ON BRAKE ASSEMBLY!!! Removing the power in these machines, either by e-stop, or sudden lose of power, should halt the machine to a complete stop.
Edit: Have you checked your error log? Check battery leads on connector sometimes these become kinda loose causing this symptom.
