Brake coming on

We've got this reach truck which is intermittently slamming on the brake throwing the driver off the seat. It has had a new steering pot and controller a few months ago which seemed to fix the problem, but I have now had it reported to me that its happened again. Has anyone else experienced and fixed this problem, if so can you point me in the right direction. Only fault codes registered are related to hyd pump motor temp E2 and a height register 86 from sometime ago.
