A Crown order picker SP3036TT210 was leaking hydraulic oil at its mast at Batam.
Two maintenance guy put 3 wooden chokes at the sides and went underneath the carriageway to dismantle. After half an hour the carriageway unfortunately dropped on top of them, killing 1 person and seriously injuring another person. I inspected the order picker after the incident and found that just a single 2 inch screw holds the central mast to the top steel frame.
In this incident this screw had given way due to it's thread being corroded.
The central round mast should have been welded to the top frame by the manufacturer. This is a major defect causing unnecessary death and injuries.
