First make sure the mounting surface of the frame is true and not bent these steer axles take a beating, and the mounting surfaces will bend if run loose, if not grind down and check with a with a straight edge or level till flat and level, replace all bolts with original or high grade allen head bolts and use nylon or steel lock nuts with lock tight, torque to spec, torquing them and locking them tight is the key
